Privacy & HIPAA Notice
Last updated October 2026
Consult Karen B is committed to protecting the privacy of every individual who calls, texts, submits a form, or attends an assessment or counseling session. This notice explains how information is collected, used, and protected.
Federal Protections
All clinical consultations, substance evaluations, and records are maintained under strict adherence to federal HIPAA privacy rules and 42 CFR Part 2, which governs the confidentiality of substance use disorder patient records. Information is only shared with a court, attorney, probation officer, or DCS caseworker when you have provided specific, signed, written consent.
Information Collected Through This Website
When you submit the intake or contact form, we collect the information you provide directly — name, phone number, email address, case type, and any details you choose to share. This information is transmitted securely and stored only for the purpose of scheduling and providing clinical services.
How Information Is Used
Submitted information is used exclusively to respond to your inquiry, schedule assessments or counseling sessions, and — with your written authorization — to prepare documentation for courts, attorneys, probation offices, or DCS. It is never sold or used for marketing purposes.
Your Rights
You have the right to request a copy of your records, request corrections, and revoke consent for information sharing at any time (except where disclosure has already occurred in reliance on that consent, or where required by law).
